PERMISSION TO PRACTICE CARD from your high school athletic office – to obtain the card you must turn in a medical (physical) form and have paid your $75 fee to your high school.
Freshman need to turn everything in to their future high school. |

All players must pay a $75 sports fee to their respective High School before they will be permitted to tryout. This $75 fee is paid directly to your High School (Freshman turn them in to your future HS). You must also hand in the enclosed Senior High Authorization for Athletic Participation form (green physical form). Your high school athletic office will then give you a blue permission to practice card. This is the card which should be turned in to your coach on the first day of tryouts.
